Home Care for Older Adults and Families Across Toronto & the GTA
When something is changing, it can be hard to know what kind of help is needed—or how much. TLC provides carefully matched PSWs and nurses, a personalized care plan, dependable scheduling and clear family communication, so the person you love can stay safer, more independent and connected at home.
Free, no-pressure consultation. You do not need to know how many hours of care you need.

You can ask before it becomes a crisis
Families often reach out when small changes have started adding up:
Daily check-in calls are becoming constant.
Meals, medications, appointments or household routines are starting to slip.
Bathing, dressing, walking, stairs or transfers feel less safe.
A spouse or family caregiver can no longer do everything alone.
A hospital discharge is coming faster than the family can organize support.
Memory changes, confusion or isolation are making life at home more difficult.
You live elsewhere in the GTA and cannot be there every time something happens.
If you are noticing changes but are not sure what they mean, that is enough to start a conversation.
Support that protects independence
The goal is not to take over. It is to understand what the person wants to keep doing and put the right support around them. We can start with the part of the day, week or care responsibility creating the most pressure—and build the right amount of support from there.

What happens next

We begin with a simple intake about the person, the family’s current situation and what a successful care arrangement should make possible. A clinical assessment follows before you commit to care.
Together, we identify the services, schedule, care team and communication preferences. The goal is the right amount of support—not more than you need.
TLC matches screened staff to the person’s needs, preferences and schedule, then makes sure the care plan is clear.
A coordinator remains involved. Care is documented, communication continues and the plan can adjust as circumstances change.
Why GTA families choose TLC

Common questions
No. Most families do not know at the beginning. Start by telling us what has changed, what is becoming difficult and what you are most worried about. We will help you think through an appropriate level of support.
Yes. Private home care can complement publicly funded care when a person needs additional hours, more scheduling flexibility, continuity, companionship, overnight support, post-discharge coverage or family communication.
TLC works to create a consistent care team whenever possible. We also provide scheduling and qualified replacement support so the family is not left scrambling when availability changes.
We begin with what matters to the person and what they want to keep doing. Support can start with practical help, meals, errands, companionship or outings and adjust as trust and needs evolve.
